Initial Consultation

Please arrive early so we can complete your paperwork. Also, please bring the following:

• Patient's insurance card
• List of current prescriptions and over-the-counter medications, including dose and frequency
• Information about patient's medical and surgical history
• Recent test results, X-ray reports, pathology reports, or relevant records.

For your convenience, you can print out our forms, fill them out, and bring them to your appointment.

• Patient registration form
• Medical history form
• Health Insurance Portability and Accountability Act (HIPPA) form.

Insurance Information

Surgical Consultants of DuPage is a provider for Medicare and most major insurance plans and provides insurance billing. Co-payments are due at the time of service. HMO and POS members will need to bring a referral from their primary care office. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
